You seem like you understand the situation beter than most. Do you anticipate reductions in sales overhead? Head count, expenses, reducing pay in a variety of ways (bene's etc.)?
are there layoffs in sales coming? cut back on the perks? how many reps are left now?
If you want to know what's next simply look to the past. It's unfortunate but Geoff appears to be repeating old mistakes. For those that don't know, Novartis launched Apligraf in 1998. It was marketed with a heavy pharma style and the doors closed on the division in 2003. It was an embarrassing failure. Now we're back to the heavy pharma and trending down. Looks like the past has come back to haunt us.

TG - it has almost come to the 3 month mark since placing you and your Merck boys on a PIP. Lets look at the improvement, Ok there isnt any. Lets look at the sales progress, there isnt any. Lets look at how motivated the field is, there isnt any (other than trying to find other jobs), Lets look at the respect that you have created, there isnt any. Don't beleive me - do a company survey. The issue is that the product is tired and old. Period. Nothing new. How do you sell more with nothing more to offer?
