Every PBM I know has it at third tier or worse. PA is a real bear and QLs allow zero dosing flexibility.

Market is going generic very rapidly
Payers don't see value
Management has ignored pain specialists and focused on oncologists as a primary market. Onc's typically have not been big writers of IR fentanyl. Although the strategy has potential, you need a Pfizer-like commercial budget to move oncologists off their asses, and that ain't happening. You get better bang for your marketing buck with pain specialists
Arch has completed no studies that demonstrate significant differentiation from other IR fentanyls
