Here’s what’s going on! Patients with diabetes and cardiovascular disease are on many, many medications. Because they refill heavily in December because they have met their deductible, prescriptions in January tend to look lower! Chicken little, the sky isn’t falling!

I agree with this. Managed care is what makes it hard. If ever script written actually converted to a fill for a patient, we would not even be having this conversation. Its getting switched and PA'd left and right. Fix the managed care and it takes off.

Managed care is where it is at! Managed care only worked in those regions of the country where they had sales reps.. We are trying to sell a med that has no coverage (previous white space). The company should have anticipated having the sNDA and had managed care work in high potential white space in late 2018 and early 2019! Pretty hard to sell when all drugs on formulary are generics!
 






Wondering how much experience current posters have with launches in post ACA world. Commercial plans have much greater flexibility for formulary changes in real time. Most Medicare plans adjust plans once per year and that tends to be later in the calendar year with changes taking place Jan 1 of the next year. Even aggressive managed care contracting rarely gets access above 50% in the first 12 months. 65 to 75% access(combined Commercial and Medicare) typically take at least 18 months. Appropriate understanding of this process can reduce stress and frustration, freeing you up to help the patients you can while the slow wheels of Medicare turn. ( I don't work for Amarin, no axe to grind). Best of luck to you all.

Understand your frustration, look at the timing of the trial and FDA indication. If the indication come after Medicare plans have negotiated and set their formularies for the next year, there is nothing to be done until the next formulary window. I know it is madding...but this is true regardless of company of its management .
 






Something to remember as well is that pharmacists at many of your large chains are incentivized to write generics. So if there is a generic alternative to your branded drug, guess what is going to happen? Docs are incentivized as well by major practices that contract with insurance companies. It’s amazing when a branded drug is written in these times.
