ROSA Over-resection

Whatever- if we make them, I want 2. It’s irresponsible to not have 2. What are you gonna do if a surgeon burns a 18 poly and turns around and and says “I need another”? “Your cuts suck” or “you have bigger problems if you need a 18 poly”- yeah right! Seriously, what are you gonna do?

mall I do is revisions with one surgeon. I have never used an 18mm poly, even in revisions. You have got to be kidding me the robot makes anyone use a size 13 or bigger. That’s a problem, only the unethical with no brains will tell you otherwise.

That's great that one surgeon you work with doesnt use thick poly. Unfortunately there's a country full of other surgeons who don't have the same skills or philosophies of total joints so you get variation all over the board

done this for over 20 years all over the country and never seen an 18mm poly or bigger EVER go in someone. Used 15mm a couple times in a revision where we were fully augmented and still had a gap to fill. You see 28mm patellas being used by a surgeon tell patients and everyone you know to run far far away. Should be criminal to be that incompetent.

18mm polys is nothing normal. You must have some Hacks as surgeons. Insane.

Who here has actually used an 18mm poly? People want doubles of a poly that should not be used? For Primary Knees? For those 18mm poly users how long is that knee gonna last? You actually believe more poly = longer life? It would seem Rosa users need some major training…. Who would make a cut without double checking you aren’t taking a massive chunk of bone with it? What surgeon would ever trust Rosa after having to use a giant poly? Worst case scenario here.

Don’t be a knucklehead! Whereas bone cuts obviously affect poly thickness, ligament elasticity is a factor as well. I see a lot of tka’s in a given year, and have only opened a handful 18mm and up polys but that is besides the point. What are you gonna do if you burn a thick poly and don’t have another? Feel free to send me any extra 18 & 20mm polys that you obviously don’t need!

I don’t even have 18mm polys in the hospital lol. You should be ashamed you use a poly that big, and learn how to template or teach your surgeons to template better. Where you at so we can tell patients to avoid you at all costs!

I’m really trying to learn here………how does templating help with the pre-op determination of a poly thickness?

your surgeon should be asking you that. The fact you are in the field and have no clue about templatimg/ligament balancing besides the fact you think they are stretched out enough to use 18mm polys…. I rarely use more then an 11mm for nasty 20-30 degree varus/valgus deformities. Did you go to training? What are they teaching these days? Insane your asking this on CP and even more insane your in the field in cases with little knowledge of what to do. Stay off CP for advice, go sit in some cases with reps who have been around awhile and ask questions and take notes. NO WAY IN HELL SHOULD YOU USE A 18MM POLY IN A TOTAL KNEE. If you find yourself there ask what the hell happened and how could have this been avoided, have that conversation with your surgeon, if he still has any trust in anything you say.

Exactly! This a soft tissue balancing operation after all. I forgot what a bunch of rocket scientists prowl around this board. I can’t speak for anybody else, but I would NEVER go into a case without a full range of polys. What kind of idiot would do that? I question your experience if you do that. And let’s also remember- we don’t put squat in, surgeons put it in. It’s our job to have it there. What are you gonna say- “I templated this case to a 13mm poly”…get back in your clown car!
