I fail to see the potential of a niche device with no data to support it, an EXTREMELY small "save" rate, shrinking reimbursement, and no mention in any guidelines!

From the outside looking in, it seems there is a smaller group of patients that would likely benefit much more than others. ICD testing started first, before the clinical trial results proved benefit in a broader group patients. Here they seemed to have started broad, before proving it really worked well in a smaller subset.

I also do believe a wearable defibrillator is in the HRS guidelines.

Note of Correction: ICD's were implanted under what we would call Secondary prevention indications today. Patients implanted early on were massive MI's with runs of sustained ventricular ectopy refractory to Class III (AMIO). Many of these patients were thought to have a life expectancy of <1yr. These devices of the time were merely shock boxes without sophisticated detection algos or ATP as we know today. Comparing a Lifevest to an Evera/Dynagen is like comparing a Model T to a Bugatti Veyron. Even by todays standards only 3-4% of AICDS fire appropriately and the majority of which have occurred >6months from implant. The 30d mortality rate is anywhere between 0.8-1.4% depending on which data set you look at. The definition for High risk has been set. The lifevest is nothing more than a "strap on AED" which is over marketed. In the interest of public surveillance, AED's have been more or less mandated and still are underpenetrated (available). The Lifevest is a bridge to obsolescences not quality care.

You must be referring to Aldo. Yup still here. Still doesn't know where the heart is..still doesn't know the customers..still doesn't know the Hospitals..still doesn't understand the reports he receives from ZOLL. Will chase 1200 dollars of Medicaid bad debt and give up 2800 of reoccurring revenue that the company screwed up. They should put him back under the rock they found him under. No second thought keep him around customers love laughing at him.
