If you had a failed fidelis/Riata/Durata you'd think differently. There are pts that refused endovascular implant that want this device. Also can you imagine if this had been around 7 or so years ago? Thousands of patients wouldn't be dealing with or wouldn't have dealt w these lead issues. that's just a couple of reasons. Once the next gen is released the acceptance will be even greater.

So the company with the best track record for cardiac leads (Reliance) are going to try to sell a "leadless" system with no reliability data, no ATP pacing and high complication rate?

This pig won't fly...as they say. In my 8 yrs of implanting thousands of devices I have never had a situation where access was an issue or the pt condition would have demanded a sub Q device. I struggle to understand giving strong consideration to implant a device with a 4 yr study of 350 pt's w/5.8% complications. The evidence of reliability is lacking and wouold be an irresponsible recommendation. The pt would have to demand it before I would consider doing the case. I believe BSX is selling against themselves and needs to be careful not to compromise their current products.

Are we going backward or forward with the technology and application of this device?

Forward. I understand the hesitation but It's the first gen. Complication rates (infection) decreased in back half of study with changes in protocol. Every company had been working on leafless technology for years. I've been around > 10 yrs and have seen Guidant protoypes of leadless Brady devices. Everyone knows its safer and better for the pt to stay out of the vasculature. The technology will only improve.

Medtronic pioneered the testing for polyurethane degradation --- of course, this was after their polyurethane leads were falling apart in the 1980's (I think it was Model 6972).

One of the basic tests is soaking in hydrogen peroxide.


St Jude knows all this stuff, not sure what the "new study" is about. From what I had seen the Aortech stuff is pretty good --- not as good as silicone from a biostability standpoint, but really, really good for abrasion resisitance.

I'd still implant a Quattro or Reliance.

Quattro? Hello NO! it's huge and outdated. The Linox (Biotronik) and Reliance (Guidant) by far the best two leads on the market. Try one

Reliance is the overall leader in reliability w quattro a close second. Period. There is 10+ yrs of data that demonstrates this. Reliance fracture rate is almost 0. The point here is stj has nothing to offer. Once Durata is pulled its over.

in our hospital biotronik has taken over as marketshare leader. we (staff) haven't seen any problems with service, prouduct, response time or procedure time. we do notice that the pwaves with their leads are usually higher than some other companies. but, basically, all the companies products seem to be good these days.

so, get your head out your ass. you were probably using IBM computers a few years ago and swore you would never use Apple. now, you have an apple phone and computer.

your industry is a commodity industry. there hasn't been a clinical advancement of much since the biV came into play. MADIT II and COMPANION were the last major studies. Those were both back in early 2000's.

we don't use Sorin but I hear their shit works just as well as Medtronic (or anyone else) and is much smaller device. Medtronic devices are huge. But, doctors don't care because they are creatures of habit.

EPs are cooky like that. Give an interventionalist a better stent and he will move from one company to the next like a hot potato.

Consider yourself lucky medtronic because you fucks have the biggest, bulkiest devices on the market.
