How is the pay structure set up for trauma 100% commission and year end bonus? Are expenses paid? In an average territory what would one bring home?

Don't waste your time. S/N used to have very nice products but have fallen behind in recent years with very little product development evident. They will also starve you for product to grow sales. $160k would be top earners. Most make $80k - $100k. With attrition of about 30% annually, they turnover their sales force every 3 years on average. Surgeons hate breaking in rookie reps over and over and it's Groundhog Day at S/N.....every day.
