Pre-employment screenings

I've worked for Pharma over the years and pre-employment drug testing used to be a urine test.
I just got lab slip with a bar code only and wonder what type of test it is...hair? Blood? Urine? I hate getting my blood drawn so its making me anxious:-(
Any One? Any feedback should quiet my fears and at least let me know what to expect. I believe I go to a quest lab.

It is a urine test.

Is unusual that the type of test isn't in the form. Just a order for drug screen with a bar code. I never saw that before. Has anyone else?

is the test usually not listed on the donor instruction sheet?
