Orphan diseases: sales reps v. MSLs

For companies that are selling orphan products with small sales forces (like Elaprase, I think, which has ~10-15 reps in the U.S.), do you need an advanced degree? Is it more MSL-like education-oriented? Or is it still standard sales reps who do detailing? Thanks.
