I got approced for it and didn’t even fill out the forms. I kept getting letters saying my dr needed to fill out paperwork for it. The problem is it takes a while for them to send the approval letter. I was afraid I wasn’t going to be approved, and returned to work. When you go back, that’s it- no longer eligible

Were they going to give you 1/2 normal severance had you been successful with documents from doctor in a timely manner?

No it was 50% of pay forever. I think your Dr just has to sign off on the form once every year or so and you just keep getting your normal check. Should have just stayed out but I was worried they were going to fire me since I wasn’t protected by short-term disability anymore

Long term is now only 6 months then they terminate your employment.
