In a novel move, Ascension Health, which is one of the nation’s largest healthcare facility operators, has banned Genentech sales reps from calling on its nearly 2000 hospitals and clinics. Why? A move by the drug company to change the way it distributes 3 of its popular cancer meds, including limiting it to 5 specialty distributors, down from the 80 that distributed the drugs previously. According to this article, other hospital groups are considering a similar move.

Gilead has a curative therapy. Compare that with onc regimens that are just as expensive (or more so) and buy you 2 more months in overall survival. Although I think Gilead is making a huge mistake in subsidizing healthcare in developing countries. (We charge the US govt $84K, but in India you get Sovalidi for $1200. Who's subsidizing that? The us!)

If more Sovaldi's are created that cure, great. The company must recoup its investment. Too many drugs are designed for chronic conditions with no cure. (Great revenue stream.) Curative therapies have a short shelf-life. We should encourage the development of more of them.

In a world of healthcare consolidation in which decision-making is increasingly be concentrated in the hands of higher level execs at IDNs, Genentech's decision won't bode well. They need to be careful about long-term effects. Sure, they'll make some easy short money. But pharma needs friends in IDNs. They are the future.
