The root cause was determined to be a Sr. cell culture Operator who was working 70 hours a week. He was responsible for signing off on the lockout tagout and missed an agitator motor that was taken off which was powered by 480V. The black (hot) and white (neutral) wires were just left hanging and the equipment was energized.

Nobody is allowed to work more than 50 hours a week now without prior authorization from management. Mandatory overtime has been implemented to distribute the overtime between all members of Team Lilly so that no individual is carrying 70 hours a week.
 








Where does the BUCK STOP? When will a CEO be held responsible. CEO's, the antithesis of a QB. When the company does well they benefit. Take a lot of the credit. Take home tons of capital. When the company does something bad or poor performance, the CEO pays no real consequence.
