Pacira isn’t shying away from the 10mL vial. 10mL is indicated for the interscaline block and orthos using Iovera really only need 10mL to hit the posterior capsule and periosteum. There is significant difference between 10mL/20mL in a TKA, twice the liposomes, I wouldn’t recommend 10mL. Pricing is closer to $225/$400 for 10mL/20mL now. It’s a shame there’s no application for Zynrelef in OMFS, that’s a very easy market to pick off. Exparel’s Ortho business will be a very easy target if it works as well as they say it does.

Hello, McFly! If we go after the 10 mL, then WE are the high-priced product. Did you really ask that question???
Also, you will look like a fool trying to get a surgeon to try to switch from a 20 to a 10 because their PI states that the 10 is ONLY for bunionectomies and interscalene blocks.
Lastly, ask yourself, “If I were going to get my knee replaced and I had the choice of getting 10 or 20 of Exparel, what would I prefer?” Don’t lie to me and say that you would prefer the 10.

Do orthopods discuss EXPAREL dosing prior to TKA? What I’ve heard is, “ We have a pain med that we inject around the surgical site to reduce your post-op pain.”
